Idiopathic nephrotic syndrome, characterized by altered permselectivity of the glomerular filter, is a common chronic renal disorder in children. Most patients are steroid sensitive and respond to therapy with remission of proteinuria (steroid sensitive nephrotic syndrome). Revised Guidelines for treatment of these patients were published recently(1). Approximately 10-20% children with nephrotic syndrome, who do not respond to therapy with corticosteroids, are classified as steroid resistant (SRNS). Their management is difficult since patients are, on one hand, at risk for complications of unremitting nephrotic syndrome and progressive renal disease and on the other, the side effects of treatment with immunosuppressive medications(2).
